What We Do

A Personal Care Assistant (PCA) is a trained worker who helps people that need support to take care of their day-to-day activities and who are on a state program (Medical Assistance, waiver programs, or Alternative Care). Personal assistance services help individuals live and work within their own communities. PCAs help people who need support to take care of daily living needs because of disease, a disabling condition, or complications related to aging.Personal care assistance services must be deemed medically necessary as determined by an assessment. The assessment must find that the person needs help to complete at least one of eight activities of daily living or exhibit Level 1 behavior.

The four categories of services that a person can receive assistance in are

Activities of Daily Living:eating, toileting, grooming, dressing, bathing, transferring (such as getting in and out of bed), mobility, and positioning

Health-related functions, which, under state law, can be delegated or assigned by a licensed health care professional to be performed by a PCA

Instrumental Activities of Daily Living: meal planning and preparation, managing finances, shopping for essential items, performing essential household chores, communication by telephone and other media, and participating in the community

Redirection and intervention for behavior including observation and monitoring

Standard Guidelines

If you choose to hire your own workers, you will also choose a PCA Choice provider (usually a home health agency) as a fiscal intermediary. The PCA Choice provider is responsible for billing the Department of Human Services (DHS) for PCA services and paying your PCA workers.
If you want to find direct support workers who may be able to assist you, go to Direct Support Connect[TM]. You'll be able to create a profile about your needs and interests, search for direct support workers, and connect securely within the site. If you are a direct support worker looking for a person/employer to support, you can also create a profile and do a search.
If you are enrolled in Medical Assistance, a waiver program, or Alternative Care, there are two possible ways that you may receive PCA services in Minnesota:

1.You may select an individual worker (where you are responsible for hiring, supervising and firing)

2.You may hire a home care agency who selects and oversees your worker

We maintain stringent standards for regulatory compliance and are ardent advocates for anti-fraud reforms.Knowledge and adherence to our compliance standards is expected of all Ebyan Health Home Care employees, and any employee with questions or concerns is encouraged to seek guidance from their supervisor.Ebyan Health Home Care engages in frequent reviews of company activities to confirm compliance with laws and regulations. Ebyan Health Home Care performs periodic clinical records reviews throughout the year to ensure ongoing adherence to the Conditions of Participation, Medicare coverage and eligibility guidelines and compliance with other state and federal regulations.

Prevention And Education

The best way to fight fraudulent activity is to prevent it. We communicate clear standards and expectations for employee activities, and require periodic, mandatory compliance training for all employees. Compliance is a consistent focus at Ebyan Health Home Care , with employees trained upon hire, as regulations change, and re-trained at least annually to reinforce compliance standards.

Eligibility

1.Be eligible to receive Medical Assistance (MA) or Medical Assistance for pregnant women and children

2.Be able to make decisions about your own care or have someone who can make decisions for you

3.Be dependent in at least one activity of daily living or exhibit Level 1 behavior within the last year, as determined by the assessment

Basic Support Services

While a patient is being medically cared for in the home, a homemaker or person who helps with chores or tasks can maintain the household with meal preparation, laundry, grocery shopping, and other housekeeping items

24-hour emergency assistance

Our Safety and 24-hour Monitoring services are designed to ensure your elderly or loved one with disabilities always have the assistance they need


Adult companion services

Companion services are an affordable, flexible, and meaningful way to help your elderly loved ones enjoy the benefits of companionship and even true friendship. It is all about recognizing how important it is for seniors to connect and nurture relationships while enjoying rich and active lives

Homemaker

Homemakers offer their clients support but do not provide hands-on care. Homemaking services allow clients to continue to live safely in their homes when they can no longer keep up with household tasks

Night supervision

Overnight assistance and monitoring provided by an awake staff in the person's own home


Personal support

Assistance with self-care, such as grooming, bathing, dressing, and using the toilet

Respite care, in home or out-of-home

Respite care provides temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member

Individual community living support

ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes
